Complete this form first to ensure all necessary departmental approvals have been obtained. You may then proceed to step 2, at which point your faculty internship supervisor will upload a scanned copy of the Sustainable Development Internship Learning Contract, with all appropriate signatures, to the Appalachian State University Internship Inventory. Keep a personal copy of this contract for your records.
- Appalachian State University Internship Inventory
Arrange a visit with your faculty internship supervisor when you can complete the Appalachian State University Internship Inventory together. The Internship Inventory may be found here – https://internshipinventory.appstate.edu - but you will need your faculty internship supervisor to access the site for you.
Your faculty internship supervisor will upload a scanned copy of the Sustainable Development Internship Learning Contract, with all appropriate signatures, to the "Documents" section of the Internship Inventory.
Once a record for your internship has been entered into the Internship Inventory, your faculty internship supervisor will generate an Appalachian State University Internship Contract. This contract, along with a copy of your Sustainable Development Learning Contract, must be sent to your on-site agency supervisor for his or her signature. In most cases, your faculty internship supervisor will send these documents to your on-site agency supervisor, but in some cases, you may need to work with your faculty internship supervisor to obtain the appropriate signatures.
A final copy of the Appalachian State University Internship contract, with all appropriate signatures, must be scanned and uploaded to the "Documents" section of the Internship Inventory. Keep a personal copy of this contract for your records.
Once the Internship Inventory is fully completed, you faculty internship supervisor will submit you internship for necessary departmental, college, and university approvals. Once approved, the registrar's office will automatically register you for internship hours. Note: If you are doing an international internship, please see the next section (G) for additional application requirements that must be met before the Internship Inventory can be completed.
